Welcome to Your First Visit With Kristoff Dental of Munster
We’re excited to welcome you to our dental office! Your first visit is an important opportunity for us to get to know you, review your oral health, and create a personalized treatment plan that meets your needs.
During your initial appointment, we will conduct a comprehensive dental exam, review your medical and dental history, and discuss any concerns or goals you may have. Same-day treatment may be possible, depending on the complexity of your medical history and the treatment plan needed.
To help us provide the best care possible, please bring the following items to your first visit:
- A valid form of identification
- Your dental insurance card, if you have insurance and would like us to file claims on your behalf
- Form of acceptable payment. Payment for services is expected at the time treatment begins.
- Copies of your most recent dental x-rays, if available
If it has been a while since you’ve had x-rays, we are happy to take new ones in our office as part of your exam.
For your safety and to ensure proper care, please notify our office in advance of any previous hospitalizations, surgeries, or medical conditions that may impact your dental care. These may include (but are not limited to):
- Diabetes
- High blood pressure
- Cancer
- Rheumatic fever
- Pregnancy
- Artificial joints or heart valves
It’s also essential that you provide a complete list of all medications you are currently taking, especially:
- Heart medications
- Bone density medications
- Anticoagulants or blood thinners (including aspirin)
